Since 1992 Adelante has provided apartments and houses for persons with mental disabilities. Our supported housing program assists mental health clients in securing transitional housing that will eventually lead to independent living. Services include: linkage with a comprehensive community support system of case management, mental health rehabilitation, respite, employment and health supports. Individuals with psychiatric disabilities, who have graduated from community residential programs or have been discharged from a psychiatric hospital, may be interviewed for the program. Clients have to exhibit basic skills that will support a high quality of life. Case Managers assist with establishing the household, ADL, budgeting and empowering consumers. They are on call twenty-four hours a day. Bilingual case management is available.

Adelante offers programs to empower and build skills among the mentally disabled population. Adelante operates a Drop-In for adult mental health consumers. We provide a comfortable environment for consumers to enjoy themselves and the company of others. The main objective is to assist consumers in developing and reestablishing a sense of self-esteem and group affiliation. The Life Skill and Vocational Training programs assist mental health consumers in becoming productive members of their community. Consumers receive self-help and empowerment interventions, community living skill education, academic enrichment and social skill rehabilitation.
The objective of a Drop In Center program is to identify and engage persons who may choose not to participate in more structured programs or who might not otherwise avail themselves of mental health services, and to provide services and supports in a manner, which these individuals would accept. These programs are low demand, flexible and relatively unstructured, and responsive to individual need and circumstance.
The objective of Life Skills program is to assist individuals disabled by mental illness to develop or reestablish a sense of self-esteem and group affiliation, and to promote their recovery from mental illness and their reintegration into a meaningful role in community life through the provision of two or more of the following: (1) consumer self-help and empowerment interventions; (2) community living: (3) academic; (4) vocational and/or (5) social-leisure time rehabilitation, training and support services.
The objective of Vocational Training is to strengthen the individual’s work record and work skills toward the goal of achieving assisted or unassisted competitive employment at or above the minimum wage paid by the competitive sector employer. TEP’s provide time-limited employment and on-the-job training in one or more integrated employment settings as an integral part of the individual’s vocational rehabilitation growth.

Adelante of Suffolk County has focused on addressing the growing needs and concerns of the community by reaching out and providing them with the necessary resources to assist in their development by providing not only community services and support groups, but also by incorporating and encouraging family structure and support. In most cases our families are hindered from participating due to the economic deprivation, and language barriers. The objective is to strengthen and develop community members that will become active leaders in the community by providing them with the necessary tools to achieve their goal. Our counselors receive extensive educational training on the issues facing the community and accessing resource guides continually so that they can better services clients needs in the following areas; Department of Social Services, Social Security, Domestic Violence, Child and Elderly abuse, Immigration, Discrimination, Housing, Education issues, Veterans Affair and Crisis Intervention.

The most pressing and immediate need is intervention on behalf of tenants to prevent homelessness and to assist those who are homeless. This includes assistance in finding affordable housing; advocacy and dispute resolution in landlord-tenant matters, assisting tenants in money management, and household budgeting. We also address the needs of tenants by referring them to programs such as HEAP, Section 8, Social Services, and Public Housing.

Adelante provides senior citizens in our community with the opportunity to enjoy nutritious and delicious meals five days a week at a very low cost of $2.00 a meal, which many are able to afford on limited income. In addition to our nutritional benefits we also offer them the opportunity to socialize among their peers, participate in-group outings; attend legislative forums in Albany focusing on issues relating to seniors, provide recreational activities, guest speakers addressing issues and concerns of seniors such as; Health Medicaid/Medicare, Food Stamps, Heap, Counseling and Low-Income Housing needs.
Adelante also provides transportation services to seniors for their medical and dental appointments; pick up their prescription drugs, shopping for food and other essential needs, and to Family and Nutrition Services.

This program was brought to fruition in 1995 as a result of the increasing disparity in the technological proficiency between low and middle-income people. It includes computer skills training, a program that started in 1994, which offers hands on training for those who are unemployed. Also included is English as a Second Language, workshops in youth leadership, self-esteem, employment opportunities, and technology to help students meet the changing demands of today’s job market.

NYS Office for People with Developmental Disabilities (OPWDD)
Adelante provides a Medicaid Service Coordinator (MSC) that helps individuals with developmental disabilities and mental retardation in exploring what they want and need in life, and assists them in gaining access to necessary supports and services. Our Service Coordinator works hand in hand with the consumer in the development and implementation of a person-centered plan to attain the consumer’s goals. The Service Coordinator assists the consumer in attaining the highest quality of life to live productively and as independently as possible.
